Title: Memory Care Coordinator
Department: Resident Care / Memory Care
Reports To: Executive Director
Status: Non-Exempt
Job Summary
The Memory Care Coordinator is responsible for coordinating the daily operations of the
Memory Care neighborhood while promoting a safe, engaging, and person-centered environment
for residents living with Alzheimer's disease and other forms of dementia.
This position serves as a hybrid role by combining responsibilities within the Memory Care
Department and the Activities Department. The Memory Care Coordinator oversees resident
engagement programming, supports quality resident care, partners with nursing and caregiving
staff, communicates with families, and assists the Executive Director in ensuring the Memory
Care program operates efficiently and in compliance with all applicable regulations and Glen
Park Senior Living standards.
Essential Duties and Responsibilities
Resident Care & Quality
1. Assist in coordinating the daily operations of the Memory Care neighborhood.
2. Promote person-centered dementia care practices.
3. Monitor residents' overall well-being and communicate changes to the nursing team.
4. Participate in resident assessments, care conferences, and care plan meetings as assigned.
5. Ensure residents receive compassionate, respectful, and individualized care.
6. Observe resident behaviors and collaborate with the interdisciplinary team to develop
appropriate interventions.
7. Promote resident dignity, independence, and quality of life.
8. Assist residents who require assistance with the activities of daily living, e.g., bathing,
dressing, grooming, eating, toileting, etc.- as needed.
9. Dispense medications, and maintain medication administration records- as needed.
Resident Engagement & Activities
1. Develop, coordinate, and implement a monthly Memory Care activities calendar.
2. Plan meaningful therapeutic programming appropriate for residents with cognitive
impairment.

3. Facilitate activities that encourage:
o Social interaction
o Cognitive stimulation
o Physical wellness
o Emotional well-being
o Sensory engagement
o Spiritual enrichment
4. Coordinate outings, entertainment, family events, and holiday celebrations.
5. Evaluate resident participation and modify programming based on resident needs.
Family Relations
1. Serve as a primary point of contact for Memory Care families.
2. Build positive relationships with residents and family members.
3. Provide updates regarding resident participation and overall well-being.
4. Assist families during admissions and transitions into Memory Care.
5. Participate in family meetings as requested.
Staff Leadership
1. Provide day-to-day guidance and support to caregivers assigned to Memory Care.
2. Assist with onboarding and training new Memory Care staff.
3. Reinforce Glen Park's dementia care philosophy and customer service standards.
4. Identify opportunities for coaching and staff development.
5. Work collaboratively with Nursing, Activities, Dining Services, Housekeeping, and
Maintenance.
Compliance & Documentation
1. Assist in maintaining compliance with all California Department of Social Services
regulations.
2. Maintain activity participation records and required documentation.
3. Ensure Memory Care activity supplies and equipment are maintained.
4. Assist with survey readiness and quality assurance initiatives.
5. Report safety concerns, incidents, or changes in resident condition immediately.
Administrative Responsibilities
1. Assist with scheduling Memory Care activities and special events.
2. Monitor activity supply inventory and submit purchasing requests.
3. Support occupancy efforts by participating in tours when requested.
4. Assist the Executive Director with special projects related to Memory Care operations.
5. Attend leadership meetings and interdisciplinary team meetings as assigned.
Minimum Qualifications

 High school diploma or GED required.
 Associate or Bachelor degree in Recreation Therapy, Gerontology, Healthcare
Administration, Nursing, Social Services, or related field preferred.
 Minimum two (2) years of experience working with seniors, preferably in Memory Care.
 Previous supervisory or leadership experience preferred.
 Experience working with residents living with Alzheimer's disease and related dementias.
 Current CPR and First Aid certification or ability to obtain upon hire.
 Ability to successfully pass all required background clearances and physical examination.

Physical Requirements
 Frequently stand, walk, bend, reach, and assist residents throughout the day.
 Lift, push, and pull up to 30 pounds.
 Occasionally assist residents with mobility during activities.
 Ability to respond quickly in emergency situations.
Working Conditions
 Assisted Living Memory Care environment.
 Frequent interaction with residents experiencing cognitive impairment.
 May require occasional evenings, weekends, or holidays based on community needs.
 Exposure to communicable illnesses consistent with healthcare settings.

Supervision Given
 Provides day-to-day direction and support to caregivers and activity staff assigned to the
Memory Care neighborhood.
 Coordinates volunteers and outside activity vendors.
