Glen Park Monrovia seeking a compassionate and dedicated Licensed
Vocational Nurse (LVN) to oversee the wellness and care of our residents in
a safe and supportive environment. Join our team at our Glendale location and

safety of our seniors!
Job Summary:
As an LVN, you will be responsible for providing skilled nursing care to our
residents, ensuring the timely and effective delivery of healthcare services,
and leading a team of care staff. You will oversee resident care activities,
administer medications, and ensure the facility maintains a high standard of
care in accordance with Glen Park’s policies, Title 22 regulations, and state
guidelines.
Key Responsibilities:
- Resident Care Management: Plan, organize, and oversee the provision of resident care activities within the facility, ensuring high-quality care and medication management.
- Care Plan Development: Develop and update resident care plans, ensuring they comply with Title 22 (DSS) & Title 17 (DDS)regulations and the facility’s policies.
- Medication Administration: Assist with med pass, ensuring timely and accurate medication delivery.
- Documentation & Reporting: Ensure proper documentation of resident condition and communicate regularly with medical professionals, agencies, and suppliers.
- Supervision: Provide leadership and supervision to care staff, ensuring the consistent delivery of care.
- Resident Assessments: Conduct admission and discharge assessments for residents, along with monthly head-to-toe evaluations.
- Health & Safety Monitoring: Monitor residents' health conditions and report any significant changes to administration.
- Staff Training: Assist with the training and monitoring of care staff on care topics and medication administration.
- Current and valid LVN license from the State Board of Nursing
- Minimum of 1 year of acute care experience
- Current CPR and First Aid certification
- Health and physical clearance within 6 months; TB test clearance
- Criminal background clearance
- Excellent written and verbal communication skills
- Ability to lift and pull up to 20 lbs.
- Ability to bend, stretch, and climb stairs.
- Ability to assist residents in standing, walking, and transferring.
